If you are a referring party from the Department of Children’s Services (DCS), Department of Corrections (DOC), Juvenile Probation,or the Department of Education (DOE), the following information is necessary to refer a potential client. All forms must be completed prior to client’s arrival.
Vision and Dental Coverage Verification
All potential clients are required to have adequate clothing. Below is an example of the amounts and types of clothing that our agency helps all clients maintain while during their placement.
